New Patient Registration
To register with us you need to collect a registration form and questionnaire from reception or download them from the links below. Once you have completed both forms, return them to the receptionist who will then process your registration. You will also need to provide 2 forms of identification to enable the practice to process and complete your registration.
Please provide identification and complete forms for each person wishing to register including children.
Patients undergoing treatment or on medication will need to book a New Patient Health Check with the Healthcare Assistant before a prescription will be issued by the GP.
PLEASE NOTE:- All new registrations must complete registration form, registration information pack and adult/under 16 registration pack.

Who can see my health record?
To treat you safely and well, it is important that professionals you see can access your health record.
Only by letting your GP surgery know it is ok will your notes be available to be seen by other professionals such as hospital clinicians, paramedics or district nurses that are involved in your care.
How do I make my record available to health professionals?
Simple. Complete this form and hand it into your GP surgery. There are two ways that this sharing can happen and it’s important you understand what they are so you can make the right decision for you.
